If you are ordering plaques for several boys in your Den or Pack, it is NOT neccessary to fill out a separate form for each Boy.
If you are ordering a plaque for YOUR son only, please use the Standard / Deluxe Order Form.

ORDER  FORM  INSTRUCTIONS
  1.Enter your Pack information       8. Select Oval or Diamond Webelos® Badge.
  2.Fill in your name & Pack Position.       9. Fill in Date Arrow of Light® was (will be) earned.*
  3. Fill in Date plaques are needed by.     10.Enter boys names. Be sure to check the spelling
  4. Fill in Den name or Number for Group # 1.     11.Repeat steps # 4 - 10 for Group # 2.**
  5.Fill in the number of boys in Group # 1.    12. Fill in "Order Summary".
  6. Fill in Date Webelos® Rank was earned.*     13.Fill in your contact & payment information.
  7. Select Deluxe or Standard Plaque Type   14.Print and Mail of Fax Order Form.
*Enter the Month, Day, and Year each rank was earned in the boxes next to the rank names.
NOTE: If exact date is not known, just enter the month & year, or the year only.
**Print as many copies of page 2 as you need to list all of your Scouts®.
